Where do I find my UCare ID number?

Your Member ID number is on your UCare ID card. Medicare and Individual & Family Plan (IFP) members: If you are using the UCare ID number from your premium invoice, add two zeros (00) after the first 7-digits in the Member ID field.

What is UCare connect dual?

UCare offers two plans for people ages 18 to 65 with certified disabilities: UCare Connect + Medicare is designed for people who want to combine their Medical Assistance (Medicaid) and Medicare under one plan.

What does UCare cover?

UCare covers a range of health care services including medical care, services, supplies, and equipment. These covered services must be medically necessary and needed for the prevention, diagnosis, or treatment of a medical condition. They also must meet accepted standards of medical practice.

How do I submit a claim to UCare?

Mail:

What is UCare PMI number?

PMI number. The unique number used to identify a person’s eligibility under the Minnesota Health Care Programs.

Is MNsure the same as UCare?

UCare is the number one plan on MNsure for Minnesotans who buy health coverage on their own.

What is UCare connect?

UCare Connect is a Special Needs BasicCare program that provides health care coverage to adults with certified disabilities. Our Minnesota Health Care Programs representatives can answer questions you may have about features of these plans.

Is UCare the same as Medicaid?

Medical Assistance (MA), is Minnesota’s Medicaid program. Medical Assistance health plans like UCare waive the monthly Medicaid Assistance deductible (the amount you must pay before your plan kicks in) which saves you money. Most people get their Medical Assistance benefits through a health plan like UCare.

Is UCare state insurance?

The state of Minnesota offers two health care programs for individuals who need financial assistance to afford health insurance: Medical Assistance or MinnesotaCare. UCare offers plans to help you access your Medical Assistance (Medicaid) or MinnesotaCare benefits.

What do you need to know about UCARE connect?

Skilled Nursing Facility: Nursing Facility Communication Form for MSHO, MSC+, UCare Connect and UCare Connect + Medicare health plans. Special Transportation Certificate of Need (Use this form if your member has mobility issues and needs special transportation that requires driver-assisted services.)

Can you register as an administrator on UCARE?

The register link above is to be completed by only one administrator within your clinic/facility. Once the administrator is given access to the UCare Provider Portal, the administrator will setup additional users within your clinic/facility.

Where can I find the formularies for UCARE?

The Formularies page on the UCare provider website shows which drugs are covered, as well as everything you need to request exceptions or prior authorization. UCare’s Provider Manual is an extension of your contractual obligations. Checking it regularly for up-to-date information and reference material is required.
