Nui Harime lost most of her consciousness from absorbing the Original Life Fiber and/or Shinra-Koketsu. With Shinra-Koketsu being absorbed by Senketsu while safely returning Ryuko back to Earth and incinerated along with the former, she is mostly dead from that point.

Yes, Senketsu dies at the end. I thought the same at a given moment : He gets into space without problem with Ryuko, but on the way back, he leaves Ryuko instead of letting her keep wearing it. This is really a “wtf” situation ! Ryuko’s landing in the episode is also another “wtf” situation.

He was murdered by Nui Harime under Ragyō’s orders, impaled by his own Scissor Blade. Sōichirō had aimed to “thwart the ambitions of the Kiryūins by using the Kamui”. His actions are the posthumous driving forces behind the plot of the series—including Ryūko’s search for her father’s killer.
